Thank you for registering to join LOUD for LIFE classes.
In order for us to complete your registration we need to collect some more information.
You can do this in 3 easy steps.
1) Please complete the following form which will ask for contact information and basic health information which we will use to help ensure your safety when participating in the LOUD for LIFE exercise classes.
2) to read the informed consent and waiver of liability document and to the mark your agreement to it.
3) to submit the application and then wait for us to send you a payment link to finalise your booking.
This process will only need to take place once. If you would like to attend multiple classes you will only need to use the payment link.
